  • The best way to schedule an appointment is by calling our office number at 805-546-8040. Our staff are happy to assist you and do our best to get you scheduled within the first couple of days of connecting with you! We are happy to provide answers any questions or concerns you may have about your diagnosis, treatment, insurance coverage, and physical therapy needs and goals.

What do I need to bring to my evaluation? 

  • Please have your insurance and driver's license available.

  • If you are being referred to us, please have your referring provider send over your referral order and any imaging reports, if performed.
